Dental X-rays: Why, Which, & When?

Digital dental x-rays have come a long way in improving our ability to diagnose problems in your teeth and jaws. But which ones do you need, when do you need them, and why do dentists recommend each kind?

Types of Dental X-rays

Periapical X-rays

Periapical x-rays capture the full length of 1 or more teeth, so that we can see the apex, or end, of the root. That’s where we look for signs of a tooth abscess, or infection. We also use them, as in this case, to make sure that dental implants are fully integrated in the bone.

Bitewing X-rays

These x-rays of the back teeth are usually taken in groups of 2 or 4, so 1 or 2 per side. Sometimes it may be a group of 7 in a different orientation. Most people have get them every 12 months to check for cavities between the teeth, where we can’t see them. If you’ve never had a cavity between your teeth and are at low risk, your dentist might change it to every 18-, 24-, or rarely every 36 months. Or if you get cavities a lot, your dentist might even recommend them every 6 months. The patient in the below example has gotten a mouthful of dental crowns in her 70+ years. Because we want to catch any cavities around the crowns early, she definitely gets bitewings once a year.

Full-Mouth Series

For patients who have cavities on many teeth, or especially if they have gum (periodontal) disease , we take a full-mouth series of x-rays. This is a minimum of 10 total x-rays, including both bitewings and periapical dental x-rays.

Panoramic Dental X-rays

The dental x-rays you see below are called panoramic x-rays. This kind is usually taken ever 3-5 years to screen for possible bone cysts, tumors, sinus abnormalities, etc. It’s also used to check growth and development in children, to make sure their teeth are developing normally. The first one shown here, which I labeled, is of a healthy patient.

But this panoramic x-ray, taken in the fall of 2019, surprised me with several unusual areas. There’s a large, dense, circular something in the right sinus, which definitely doesn’t belong there. There are also 2 irregular areas of unusually dense bone on the lower jaw. We have sent this patient to an oral surgeon for further evaluation.

Because we wanted more information about those unusual areas, we took another kind of dental x-ray, described next.

Cone Beam X-ray, or CT Scan

When we need a lot more detail, or different perspectives and angles, we take a CT scan, also called a cone beam or 3D x-ray . Cone beam images are quite different from the other types, because they’re taken in slices. Computer algorithms turn those slices into a 3D images. That 3D x-ray can be rotated in every direction, and we can scan through it up/down, forward/back, and side-to-side. They’re really quite incredible!

These cone beam views are of the same patient above, with the panoramic x-ray. Now we see the true size and shape of those unusual areas.

3D X-Rays for Dental Implants

When planning dental implants , we have to know if the bone is both wide and tall enough. If it is, great! If not, then you may need a bone graft or sinus lift to build up the bone. Without the 3D views, we can’t know that. We can also see other important anatomy, like the big nerve in your lower jaw (the yellow line). We really don’t want to run into that!

In short, dental x-rays give us critical information about both your teeth and your jaws. Without them, it’s really impossible for us to do a good job for you.

What is Root Resorption? Can it Be Fixed?

Root Resorption comes in 2 flavors, so to speak: Internal and External. It’s one of the least common reasons for losing a tooth, compared to tooth decay and gum disease. When it is found though, it usually means the tooth is doomed. But what is root resorption?

Internal & External Root Resorption FAQs

The term “resorption” comes from the Latin root resorbere , meaning “to absorb.” In teeth however, it’s not really absorption like a sponge absorbing water. A tooth being resorbed is actually eaten away , either by cells inside the pulp or outside the root. Because the growing cells shouldn’t be, resorption is a little like cancer. Only a little bit though. That’s because root resorption never spreads, not even to the tooth next to it.

Internal Root Resorption is caused by Pulp Cells

Pulp cells are found in the nerve chamber and canals inside the tooth. They start growing outward, turning the dentin into a sponge-like, soft, mushy mess. It’s usually a very slow process, and it’s rarely painful until it has progressed a lot. Because it usually happens in tooth roots, it’s often not visible on normal x-rays. That means dentists can’t see it, unless there’s a reason to take an x-ray of the whole tooth, as seen here. This was taken in July 2019, on a new patient in our practice. It became sensitive just recently.

Based on this x-ray alone, I didn’t know if this was internal or external resorption. So we took a 3D Cone Beam x-ray , to see it from multiple angles, as seen below. From that, it appears to be internal root resorption. It got big enough to break through the root, which allows bacteria in. That’s probably why it became sensitive recently.

We don’t know yet if this tooth can be saved, but I’ve referred her to a root canal specialist (endodontist) and gum specialist (periodontist). If they can’t save it, we will probably do a dental bridge to replace the tooth. The other tooth roots are too close together for a dental implant . I’ll try to remember to update the post once she makes a decision.

External Root Resorption is caused by Periodontal Ligament Cells

As I mentioned, root resorption is pretty uncommon. In my 20+ years of practicing dentistry, I’ve only come across a few cases. That’s why the patient in the first case above was so unusual. When evaluating the lower front tooth resorption, I noticed a second tooth with resorption, but this time it clearly started outside of the tooth. 2 teeth with resorption on the same patient is odd enough. One tooth with internal resorption and one tooth with external resorption on the same patient??? I’ve never seen, nor even heard of that happening.

It’s a good thing we took the 3D Cone Beam x-ray on the patient though, because we couldn’t see it on the regular bite-wing x-ray at all. This tooth didn’t bother her at all, so we had no reason to suspect it.

Unfortunately, this tooth is definitely not fixable, so it will be extracted and probably replaced with a dental implant and crown. Since it isn’t bothering her and isn’t visible in her smile, we’ll wait until the other tooth is finished.

Inflammatory Root Resorption after Braces

There is one type of root resorption that is unfortunately a bit more uncommon. This kind occurs after aggressive tooth movement, and it seems to have a partially genetic basis. This panoramic x-ray is from a long-time patient in our practice. Her roots have gotten progressively shorter over the years, and now they’re starting to get loose. It’s tragic, but she will probably lose all her teeth within the next couple years. That’s why we’re discussing implant-supported bridges to replace all her teeth soon.

Repairing a Broken Dental Bridge

If you have a dental bridge and some of the porcelain has broken off in a place visible in your smile, the bridge usually has to be replaced. Sometimes however, the bridge can be repaired with a creative and nifty technique called an “overcrown.” This isn’t always an option, but when it is, it’s a great way to save you money and time at the dentist.

Repairing is Less Expensive than Replacing

Our patient M., as we’ll call her, has been with our practice since late 2006. She had some well-done dental work many years before, including long PFM (Porcelain-fused-to-Metal) dental bridges. In the spring of 2008, however, she broke a big piece of porcelain off a top front tooth. The bright silver shining through really bothered her. She didn’t want to replace the whole thing, because it cost too much. I’d recently learned the “overcrown” technique through a professionals-only dental forum. Given it was the only option except for replacement, she let me try. I think you’ll agree – it turned out pretty well!

The basic process was similar to any other crown. I drilled off the remaining porcelain on the broken portion of the bridge to expose the metal. A margin, or edge, was shaped along the gumline, where the crown stops. We made an impression, or mold, which is something we almost never do anymore, thanks to the CEREC Bluecam and the TRIOS 3 Intraoral Scanner). We cemented a temporary crown for 2 weeks. When the real crown came in, we cemented it like normal.

As of this blog publication date, that bridge repair with an overcrown is 11 years old, and it still looks great.

CEREC Emax Overcrown to the Rescue

This case is more recent & was completed in March 2019. This gentleman’s Maryland bridge, as it’s called, was more than 30 years old , and still in (mostly) great shape. Unfortunately, he fell and hit a front tooth, breaking off a big piece. For perspective, that bridge was done 9 years before I became a dentist. It was done the year before I started college! The oldest and still functioning dental work I’ve done that I know of, is a 19 year old CEREC crown for my Mom. �� Anyway, back to the story…..

Given the bridge is still solidly cemented and functioning, I sure didn’t want to replace it. The patient didn’t want to spend that much money either. So I suggested an overcrown for him, too. This time, we did it with with our CEREC Bluecam CAD/CAM system. True, it is “old “technology, but it’s still among the best available. We chose a material called Celtra DUO for its strength, esthetics, and because it matched color so well.

Maybe one day, he’ll let me do the same for the tooth next to it, to take care of the chipped corner. He said it doesn’t bother him though, so we left it alone.

The process was similar to the previous case, but instead of taking a mold and sending it to the lab, we sprayed a thin coat of blue powder over the teeth, scanned it, designed it on the Bluecam, milled it, polished it, and bonded it in. The whole process lasted about 75 minutes. No numbing, only a little drilling, and a little polishing afterwards. Not too shabby, if I do say so myself. ��

Why Can’t Dentists Cure Cavities or Regrow Enamel?

I’ve previously discussed why you can’t cure cavities , but recent research sounds promising, so it’s a good time to review. In addition, there were some good points raised in the comments of the previous article that deserve more attention. Be warned: lots of bullet lists ahead as I break it down. ��

Recap: Why We Currently Can’t Grow Dentin or Enamel

Let’s briefly summarise the difficulties that must be overcome in order to cure cavities, whether naturally or artificially:

Once the cells that form dentin (odontoblasts) and enamel (ameloblasts) finish forming teeth, they die. Some odontoblasts remain in the tooth pulp, but they are stuck inside and can’t get out.
We currently have no way to grow and transfer dentin- or enamel-forming cells into a tooth with a cavity and survive, much less grow new tooth structure.
The dentin-forming cells in the pulp can’t grow outwards, just inwards, which shrinks the pulp chamber. This is a normal response to stress from cavities, bruxism, trauma, and aging.
If you already have a cavity, we still have to get rid of the bacteria, which usually means drilling out the infected tooth structure.
If we don’t want to drill, but just put a miracle gel on the tooth, it has to do 3 things:
- Stay in place for a long enough time to strengthen the enamel and soak way in for deep cavities
- It has to kill the bacteria inside the cavity
- It can’t negatively affect the rest of your mouth.
If we have to drill out the cavity, then place the miracle gel, we still have big issues:
- How do we control how much dentin and enamel grow? IOW, it has to grow the right amount of dentin and/or enamel, and then stop. If it keeps going, that’s bad!
- How fast will the dentin and enamel grow? Let’s say it’s a big cavity, will the gel take an hour? A day? A week? A month?
- If it takes longer than the numbing lasts, how will we keep the sensitive inside part of the tooth from hurting?
- If it takes more than an hour or 2, how will we protect it from from saliva and food and bacteria?

Put simply: after a tooth is formed, the cells die off, leaving just the crystalline enamel and mostly crystalline dentin. When a cavity starts and forms a hole in the tooth, there aren’t any cells there to regrow the enamel, and the dentin-forming cells inside the tooth are very limited. Even if we figure out how to get the right cells and enzymes and minerals there, there are still more complications.

What Research Has Been Done On Growing Dentin & Enamel?

A few studies keep getting mentioned every time the subject comes up. To be clear, I am only including reputable research from legitimate sources, not absurd nonsense from something like the Weston Price Foundation. There are more studies than this, but they are just variations on these.

Grow Your Own Teeth After 50 (Kings College, London, UK 2009): this research used stem cells to regrow entire teeth in mice, not to repair cavities.
No More Fillings as Dentists Reveal New Tooth Decay Treatment (Kings College, London, UK 2014): this study used a low electrical current to force minerals into enamel weakened by decay but not completely broken down. Shows some promise and has limited testing in humans.
Scientists have Figured Out How to Regenerate Teeth with Lasers (Harvard, 2014): this study used lasers to stimulate stem cells in a lab to grow dentin and enamel. It has not been tested in animals or humans yet.
Researchers Grow Teeth From Gum Cells (Kings College, London, UK 2013): this study combined human gum cells with mice cells to grow whole human teeth. It was only done in lab dishes, only to grow whole replacement teeth, and not rebuild tooth structure lost to decay.
Laser Therapy Prompts Regeneration in Teeth (the NIH and Harvard, 2014): This was a lab dish study showed that low-level laser stimulation can trigger growth of dentin from rat stem cells. Nothing in humans yet.
How to Eliminate Pain Tied to Tooth Decay (U. of Southern California, 2015): 2 separate studies at USC by different groups. This is the most promising research, as it addresses the challenges of “how do we grow enamel if there aren’t enamel cells and there’s a big hole in a tooth.” Nevertheless, it is still only in a lab dish and has not been tested in animals or humans.

Looking at these 7 studies, a few things stand out:

Most were done in lab dishes on tissue samples and not in animals or humans.
The only one tested in humans was from Kings College, which involved forcing minerals into teeth weakened by cavities, not with actual holes or big cavities.
Most involved stem cells to grow entire teeth. That is totally different from getting enamel to grow in a tooth in someone’s mouth while they’re using it every day.

Maybe We Can Cure Cavities in 10-20 Years

In other words, none of these are remotely close to being available to cure cavities now. Except for the one that uses an electric current to force minerals into weakened areas, none will even be tested in humans for another 5-10 years. Will any of them work in the mouth, since not all lab research works in real life? Even if it does, how expensive will the process be? Will it be available over the counter or require a trained dentist to perform? Will it require big lab equipment or be a little kit you can order online? We just don’t know.

What about Better Dental Sealants?

In the comments on that earlier article about why cavities can’t be cured or “healed”, questions were raised about why we can’t invent something that will coat the teeth and protect them from cavities. After all, we do have dental sealants to seal the deep grooves on the chewing surfaces of back teeth, so why not something to protect the whole tooth? This would be a game-changing breakthrough, but as with a way to heal cavities, there are some challenges. Here’s a partial list of the characteristics such a sealant would need:

Non-toxic
Invisible
Able to withstand chewing forces of hundreds of PSI (pounds per square inch) thousands of times per day for many years
Resistant to temperature extremes like the heat of coffee or the cold of ice cream
Resistant to the acids and bases present in food and drink
Not trigger an auto-immune response
Resistant to the bacteria and by-products normally found in the mouth
Easy to apply
Only cover the teeth, not the gum tissue
Microscopically thin to not interfere with teeth fitting together

As I told the person who asked, if anyone develops a material that does all of those things, that person will be rich beyond belief. If it is safe enough to sell over-the-counter, the profession of dentistry will die the day it comes out. Well, except for the fact that we’d still have to solve gum disease, too, which is a completely different set of bacteria.

Could We Make a Vaccine Against Cavities?

On the surface, it seems a vaccine against the bacteria involved in tooth decay and/or gum disease is much better option. In searching for research on dental vaccines, there hasn’t been a lot of it, and most has been in animals. Sadly, the consensus seems to be that the financial investment required to fully fund the research and develop a vaccine is too great. In an article published in January 2014, Dental Caries Vaccines: Prospects and Concerns , author Daniel J. Smith, PhD, summarizes well (added emphasis is mine):

There are other problems, of course. The dental biofilm is the microscopic layer of bacteria and connecting material that coats your teeth, and it’s a very complicated bacterial mattress, so-to-speak, with lots of bacteria, and more than one of them contribute to tooth decay. That means a vaccine has to cover more than 1 bacteria to really work well. Also, since biofilm is pretty hard to disrupt or penetrate, any vaccine has to overcome that barrier, too. So unfortunately….it’s very difficult, which means developing a vaccine will be very expensive, and we still don’t know how well it will work, even in animal models.

What’s That “Silver Diamine Fluoride” Stuff?

In early July 2016, the New York Times published an article titled A Cavity Fighting Liquid Lets Kids Avoid the Dentist’s Drill . In it, they discuss a product called silver diamine fluoride, or S.D.F., which has been used in Japan for decades and was approved for use in the USA by the FDA as a tooth desensitizer in 2015. Pediatric dentists are using it off-label as a way to stop early-stage cavities and reduce risk of more cavities in children. It’s only available from one company in the US, Elevate Oral Care, under the name Advantage Arrest. The ADA put out a press release the day after the NY Times article, too. So……….does it work?

From online professional discussion groups, it seems the consensus is: yes, it works. It can stop early-stage decay and seems to reduce the risk of more cavities forming. The downsides? It stains the areas of demineralized, weakened enamel black, and it’s not covered by dental insurance yet. On the flip side, it’s fairly cheap. The University of California at San Francisco (UCSF) published a paper on silver diamine fluoride that describes the product, how it works, its limitations, and current recommended protocols.

Here at Smiles by Payet Family Dentistry , we are still evaluating the evidence and talking with pediatric dentists especially, to see if we should start offering this product and will make a decision soon.

Conclusion: Nope, We Can’t Cure Cavities Yet

After all that, the best thing you can do is to prevent cavities. And we already know how you can do that: reduce the amount of sugar in your diet, eat more vegetables and fruits, choose whole grains, avoid acidic sodas/energy drinks and even too much juice, brush with a fluoride toothpaste and floss (yes, you still need to floss), and see a dentist for regular checkups.

When everything is said and done, Prevention is Still the Best Medicine !

Replacing Veneers after 15 Years

There are a number of important questions to ask when considering cosmetic dentistry, including porcelain veneers:

How many veneers do I need?
What can (and can’t) you change about my smile with veneers?
When are veneers better than braces?
How much do veneers cost?
How long do veneers last and what happens when they need replacing?

Recently, we saw a patient for whom I did 8 porcelain veneers in 2001, and his case illustrates all of these questions, so without further ado, let me introduce Steve’s teeth. (He’s a rather private person and prefers not having his portrait used, and this is something that we always respect.)

Please forgive the varying levels of photography you see through this article; the first photos were originally slide film that were scanned from negatives, then they move through a series of progressively better cameras and a progressively improving photographer (me). �� I’ve tried to even them out with Photoshop, but rest assured, absolutely nothing else has been done to the photos.

8 Porcelain Veneers in 2001

Steve originally came to us because he was getting ready to attend a big high school reunion, if I recall correctly. It was on kind of short notice, too, as in he was catching a flight the next day. Yikes! But we arranged to see him later that day and started him on 8 veneers to lengthen his worn-down teeth. He looked pretty good with the temporary veneers for the reunion, and we saw him a few weeks later to bond in the real porcelain veneers. So back in 2001, the first set of 8 veneers took Steve’s smile from here:

Fast Forward to August 2015 & 2 Cracked Veneers

Given that Steve has a very demanding job that requires a lot of travel, we didn’t see a lot of him after the first veneers were done. Then he moved out-of-state and we obviously didn’t see him at all for many years. Imagine my surprise, when I got a text message on a Friday evening in August that 1 front veneer had broken and another had a crack in it! Fortunately, Steve still did business in Charlotte and was on his way here when it occurred. The tricky part? He was leaving for a big vacation in just a week. So the only thing to do was to see him on a Saturday morning to fix him up. As it happened, a 3rd veneer had a crack, too. After some discussion, we decided to re-do those 3 veneers for now, and we’d consider whether or not to redo the 4th front tooth if it didn’t match just right.

So why did Steve have 3 cracked veneers? A simple but powerful reason: bruxism associated with sleep apnea. Steve was diagnosed with sleep apnea a few years ago and had started wearing a snore guard that doubled as a nightguard. But once porcelain is stressed, it can’t ever be unstressed, so the years of bruxism before the snore guard took their toll and eventually the porcelain cracked. Also, his lower front teeth were already worn and uneven, which also put stress on the porcelain. Veneers on the lower teeth would be an option to reduce that risk, but they’re not enough of an issue for Steve to do.

This is why we always recommend a nightguard to protect your investment in cosmetic dentistry!

A Commitment To Patient Satisfaction, Even Under Pressure

While we typically work with an boutique dental lab for our big cosmetic cases, there was no time for that in Steve’s situation. Our local lab worked super hard to get the 3 veneers done as fast as possible, and we saw Steve the next Thursday at lunch to bond them in. However, the first set honestly did not look too good. In fact, they were kinda bad. Like this:

With a lot of photos and notes, we returned the case to the lab for a rush fix, and they did a far better job this time! We saw Steve back the very next morning, Friday at 8am, to bond the improved veneers in before he left for the airport. Seriously. We bonded them in, checked his bite, and he drove to the airport for his flight. No pressure! He looked a heck of a lot better like this at that point:

But while the 3 new porcelain veneers did look much better, Steve and I ultimately agreed that we should have done the 4th front tooth for the best match, so early in 2016, we started redoing the 2nd right front tooth look like the other 3 front teeth. Even then, though, Steve had a few small gripes about the other 3, and to be honest – he was right. So after more discussion by text and phone about his ultimate cosmetic goals, we agreed that the right thing to do was to redo all of his veneers and even add the first molar on each side. This time, we would work with our top lab, because Steve wanted only the best. The new goals included:

Brighter white veneers
More natural texture, not so flat-looking
Maintain the masculine, squared-off look that Steve preferred
Make the smile broader by adding the molars and giving them more bulk to fill out the smile

Veneers Can Last for Decades and Rarely Fail All at Once

Steve’s case is fairly typical in a number of ways and illustrates a number of things about the durability of veneers, as well as the reasons they fail:

Properly bonded porcelain veneers can last 15-20 years and longer
Just as with most things, they rarely fail all at the same time. It’s usually 1-2 at a time. This means you can replace them as needed.
- Steve chose to redo all of his veneers because he wanted all of them even brighter than the first set.
- If the rest of the veneers are ok, there is no need to redo all of them.
Don’t forget: as long as you have teeth, you can get cavities. Brush, floss, and see your dentistry regularly!
If you’re investing $15-40,000 in your smile, wear a nightguard. It’s the cheapest and most effective insurance available.

Hopefully this article has provided answers to many or all of your questions about porcelain veneers, their durability, what happens when they need replacing, why they often need replacement, and how to care for them so they’ll last as long as possible. I do have other patients with veneers that we did 15-16 years ago that are still going strong and not in need or replacement yet; I’m looking forward to seeing how much longer they last!

Dental Cavities: How Fast Can They Grow?

How fast dental cavities grow may sound like a simple question, but it’s actually rather complicated. I decided to write about it, though, after seeing a patient recently to treat a cavity that went from just beginning to an almost-root-canal-sized cavity in just 14 months. It’s a good example of why most dentists still recommend dental bitewing x-rays every 12-18 months.

Is the Cavity There or Not?

Here’s the starting x-ray, taken on Dec. 14, 2014 . If you look at the circled area, it looks fairly normal. Unfortunately, there was a slight overlap of the teeth in the x-ray, as the digital sensor was angled ever-so-slightly, and the overlap may have masked the area of concern, preventing an earlier diagnosis. Since the patient had few fillings, though, indicated a low risk of cavities overall, we didn’t retake the x-ray to avoid unnecessary exposure. Nevertheless, there’s really nothing there that caused me any concern.

14 Months Later, the Cavity is Obvious & Deep

Check out the circled area this time, eh? I’ve outlined the nerve chamber in the tooth on the x-ray in red and the cavity in green, but it’s important to know that a cavity is never sharply defined like this. There are several zones identifiable in cavities, highlighted in the next image. We also know that cavities are typically about 20-30% bigger than can be seen on an x-ray, which is due to 2 reasons:

The human eye can only distinguish about 60% of the shades of grey as a computer monitor can project, so there is simply detail that the eye can’t detect, and
The deepest zone of decay has not yet softened enough to be less dense enough for x-rays to detect it, as dental x-rays are all about levels of density. Darker black areas mean it’s extremely soft and x-rays go right through without being blocked, while bright white areas indicate that x-rays are being completely stopped.

Dental Cavities Aren’t Always Obvious from the Outside

In this short series of photos, taken through our Leica Dental Operating Microscopes at 8.5x magnification, you can see the progression as I clean out the cavity. On the surface, you can’t even tell something is wrong, can you? But once the tooth is opened up, the mushiness is obvious. And when you see how deep the final cavity is, hopefully it also makes sense why dentists tell you that it’s impossible to heal, or reverse, deep cavities, especially with diet.

Not All Dental Cavities Grow Fast. What are the Differences?

As it turns out, there was a good reason that this patient’s cavity grew as fast as it did: the patient had developed a habit of sucking on cough drops due to dry air in his workplace making him cough. He would place the cough drop between his cheek and the upper right molars, and since they weren’t sugar-free cough drops, the long-term sugar exposure in that one area provided the bacteria all the food supply needed to grow rapidly. Obviously, not all cavities will progress at this rate. Sometimes they will progress incredibly slowly , taking years to even break through the enamel and require fixing. If they haven’t broken through the enamel, they might even reverse. Cavities as deep as illustrated, though, can never reverse. They have to be fixed.

Does Everyone Need Dental X-rays Annually?

Most definitely not! While many dentists find it easiest to maintain all their patients on annual bitewing x-rays for consistency (and there’s really nothing wrong with that, as the level of x-ray exposure from dental x-rays is unbelievably small), this is no longer the ADA’s or FDA’s recommendation. In 2012, these organizations developed new guidelines for dental x-rays ; while the ultimate decision is still up to the treating doctor, the recommendations allow more flexibility in extending the interval between x-rays, depending on various conditions. The guidelines generally group people into one of three categories, with the corresponding recommended frequencies (these are all for teens and adults; I’ll cover recommendations for kids in an upcoming article) :

New Patients (teens and adults)
- Selected x-rays to visualize areas where decay is most likely to occur.
  - This usually includes bitewing x-rays for the back teeth and 2-4 periapical x-rays of the front teeth
  - Patients previously or currently diagnosed with periodontal (gum) disease should have a full-mouth series (10-18 x-rays to cover all teeth)
Current Patient (teens & adults) with HIGH Risk or CONTINUING Risk of Dental Cavities
- Selected x-rays every 6-12 months of all areas where decay has recently occurred or is likely to occur
Current Patient with LOW Risk of Dental Cavities
- Routine bitewing x-rays every 18-36 months
  - Quite frankly, the large majority of dentists disagree with 18 months being the routine interval, for the very reason illustrated in this article.
  - In our office, we are moving a number of patients to 18-24 month intervals, but every 3 years? Sorry, I’m simply not comfortable with that, because so much can change in 3 years. I don’t actually know of any dentists who do allow up to 3 years between x-rays, although there may be some out there.

We Do NOT Discriminate

Let me start off by saying that, while I’m not a native North Carolinian, I have lived here for almost 26 years and have deep family roots in the state, as my Mom was born in Lewisville and grew up in Albemarle (most of our extended family still live in the state, too), and my parents met in Chapel Hill during college. I chose to start my practice here and live here with my wife and daughters. Charlotte, NC has been my home for the last 18 years & will probably be our home until we die.

However, since the passage of House Bill 2 by the NC Legislature and Governor McCrory’s signature on Wednesday, March 23rd, 2016, I am now deeply ashamed to say that I am from North Carolina. And while I generally try very hard to avoid discussing politics within our practice, in person or in writing, I believe this is a time to stand up for what I believe is right, both personally and professionally. The following article will describe why I believe that HB 2 is wrong and the NC Legislature should repeal HB2 immediately:

Repeal HB2: It is Immoral & Discriminatory

First and foremost, I believe that our state legislature and Governor have now signed into law a measure that makes North Carolina among the most discriminatory and anti-LGBT in the nation, and that is wrong. The United States Constitution and long legal precedents have determined that ALL people are deserving of equal rights, not just the ones approved of by any single constituency or any religion. As is often said, “Rights can’t be voted on; that’s why they’re called RIGHTS.” But the NC Legislature and Governor have now put into effect a law that very specifically discriminates against certain individuals in our society. The fact that they are trying to justify it based on moral grounds is equally reprehensible and disgusting to me. It deeply saddens me to see the discrimination and hate of the past, previously focussed on women, African-Americans, Catholics, Native Americans, or Jews (among others), now revived against LGBT individuals. Regardless of what religion you may or may not follow, I believe that all humans are deserving of equal treatment simply because it is just and right.

HB2 Eliminates Discrimination Claims in State Courts

This is something that many people may not understand, but which has potentially drastic consequences for anyone who has experienced discrimination and wants redress from the courts. Even if you are a member of a protected class, HB2 has now eliminated the ability to file suit in state courts. This includes all of the following:

Gender discrimination
Religious discrimination
Age discrimination
Racial discrimination

Here’s a perfect example happening as we speak. A NC woman suing for age and disability discrimination has now learned that her case can not go forward, and she must try to file suit in federal court, which is much more expensive, takes much longer, and there is no option to receive damages for pain or suffering. To make my point on how bad this is for North Carolinians, consider this.

I could

fire one of my employees for having tattoos,
fire another for being Asian, and
fire another for being “old,”
even fire one of them for being Christian

and because of HB2, not one of them could sue me in NC Courts.

Now to be perfectly clear – I have absolutely no intention of firing ANY of my Team members, because we value all of them for being wonderful human beings and hard workers who take great care of you. Nor would I be such an a**hole to ever fire anyone for any of those reasons. But my point is that I could, and they would have no recourse in North Carolina courts thanks to HB2.

HB2 jeopardizes BILLIONS of $$$ to Schools & Transportation

In one fell swoop, our Legislature and Governor have put at risk over $4 BILLION in Title IX funding for our schools from the US Dept. of Education, another $1 BILLION dollars in funding for our public infrastructure from the US Dept of Transportation, and perhaps more billions from the Dept. of Housing and Urban Development (HUD). To foolishly endanger that kind of funding for our schools, roads, and public housing, which still have not reached the levels of funding prior to the Great Depression of 2008, is flat out stupid. My older daughter attends NC State University, and my younger daughter is in 3rd grade in Charlotte-Mecklenburg Schools, both of which would be devastated by the loss of federal funds. Sure, I can afford to take my daughter out of public schools and pay for private, but what about all the other children who absolutely depend on public education? And I personally like having safe bridges and roads and infrastructure, and many of our state residents depend on public housing that depends on those federal funds.

Quite frankly, to jeopardize the education and infrastructure of our state in the way that the Legislature and Governor did, is unbelievably irresponsible. It would be utterly impossible for the State of North Carolina to make up those funds if they were lost, which would destroy the educational system, allow our infrastructure to decay, and potentially force many into homelessness. What kind of people would take such risks, and are they worthy of governing? Clearly, my answer is NO.

HB2 is Damaging our State’s Reputation with Businesses

As of this writing (and I will update this as events unfold):

more than 120 CEOs of major corporations, including Pepsi, Apple, Facebook, Google, Hyatt, Qualcomm, and Starbucks, have signed a letter to McCrory protesting House Bill 2.
The NBA has stated that it will reconsider hosting the All-Star Game in Charlotte in 2017. [Update April 15th, 2016] the NBA has said it will not move the All-Star Game out of Charlotte, but will continue to work at repealing HB2, which it believes is a bad law.
The High Point Market has received many cancellation notices that will hurt business. (If you don’t think that’s a big deal, the High Point Market is the single largest economic event in the state, responsible for up to 600,000 visitors and over $5 billion in the state’s economy.)
The NCAA will reconsider hosting future tournaments in the state.
Google Ventures is banning investments in NC tech startups until HB2 is repealed.
Multiple state and municipal governments around the US have banned travel to North Carolina.
Pharmaceutical company Braeburn is “re-evaluating” a planned $20 million project in Durham.
Film and TV studios are cancelling future projects in NC
Tony Award-winning producer Stephen Schwartz is forbidding production of famous musicals in NC , including “Wicked,” “Pippin,” and “Godspell”
[Update April 5th, 2016] PayPal is withdrawing plans for a Charlotte expansion This will cost North Carolina 400 high-paying jobs.
[Update April 5th, 2016] Red Ventures CEO Ric Elias just sent a letter to Governor McCrory, stating that RV may move up to 500 future jobs out of North Carolina

Simply put, this ill-conceived bill has cost our state billions in lost business, hundreds or thousands of well-paying jobs, and millions in tax revenue. For our state representatives to pass a law that could have such drastic repercussions was bone-headed and moronic at the very least, but what is JUST as bad now, is the way they continue to dig in their heels and blame everyone except themselves for a horrible law.

HB2 Made a Mockery of the Democratic Legislative Process

For them to have done it in the manner that they did – from introduction to being signed in less than 12 hours with virtually no chance for debate or modification, is another reason I oppose House Bill 2. Disgusting. Absolutely disgusting. And the reason for it is grossly obvious – to fire up the conservative GOP base of voters in an election year. I’m not going to go into this further because it’s self-explanatory. It’s wrong to abuse legislative power, which is exactly what they did. And before someone points out the obvious – if Democrats have done anything like this, anywhere, anytime – yes, it was wrong then, too.

Democratic state Representative Billy Richardson (who voted for HB2), now says that he regrets his vote and will be supporting legislation in the upcoming session to repeal it completely.

HB2 is Based Almost Entirely on a Lie

The next reason I oppose the “bathroom bill,” as it’s come to be called, is for the simple reason that it’s based on a lie. The lie is that this law is necessary to protect women from men who just pretend to be transgender to get access to women’s locker rooms, changing rooms, and bathrooms in public and/or private facilities in order to sexually assault women and girls. But PolitiFact North Carolina, an independent fact-checking organization, together with the News & Observer, investigated the claims that such assaults have happened and have found almost no instances of it happening anywhere . 15 Experts Debunk the “Transgender Predator” Myth

The simple truth is that 17 states and 200 municipalities in the USA have had laws like Charlotte’s ordnance for up to a decade, and the “predatory transgender person” is simply a myth. A lie. A Boogeyman to scare you. But like the Boogeyman, it doesn’t exist.

Let me be perfectly clear. I have 2 daughters, one 21 years old and the other 9 years old. I am not afraid of this happening.

Do I fear that one or both of my daughters could be the victim of sexual assault? Of course! It’s every father’s worst nightmare. But I have no fear that a transgender man who identifies as a woman will ever end up assaulting either of my daughters in such a way that this law would make a single bit of difference. It’s a nonsensical, imaginary argument based on fear and not on reality. I am far more scared of the reality that they could be assaulted by a “normal” heterosexual male.

BTW – Who Will Enforce HB2?

Forgive me for being really sarcastic for a moment, but given that the Legislature just created a new type of crime, how exactly do they plan to enforce it? Seriously, how will they?

And how will transgender women, if they haven’t changed their gender on their birth certificate, manage when they now look like this? Who’s going to check when they walk into the women’s bathroom? Or if a transgender man, legitimately dressed as a woman, walks into the men’s room? Somehow, I don’t see that working out too well.

HB2 Was and Will Be a Huge Waste of Taxpayer Money

First of all, the Legislature actually called a special session to ramrod this bill through, and it cost them an extra $42,000 to do it. Why? They were already coming back to session in April anyway; they couldn’t have waited a few weeks? Hey, it’s not like they called special sessions for teacher pay, or funding for schools, or anything that might actually benefit all North Carolinians, but they could do it for this?

Not only that, as was completely predictable, HB2 is being challenged in court by several organizations, including the ACLU, Equality North Carolina, and Lambda Legal. Since current NC Attorney General Roy Cooper has declined to defend the law on behalf of the state, stating that it is unconstitutional, the state will now have to hire outside attorneys and pay for them. It does seem likely that this will end up before the Supreme Court, and the lengthy legal challenges will be expensive.

Does HB2 Do Anything Good? No, It Doesn’t.

Quite frankly, I see absolutely zero benefit for citizens of North Carolina. I really don’t. All I see is a law that was ramrodded through in record time, that is highly discriminatory, and will cause tremendous financial damage to our state. Numerous other states have recently stopped similar laws (Georgia, North Dakota, Iowa among others) once their Governors realized just how badly their state could be hurt. With the almost instantaneous passage of HB2, there was no time for reflection on its (total lack of) wisdom.

The question now becomes: just how bad will it have to get, and how many North Carolinians will have to suffer temporary or permanent harm, before our legislators and Governor undo this monstrosity? Or will the Courts save us first?

Before we start talking about how many porcelain veneers or crowns you need/want, let’s talk about when cosmetic dentistry is best and when braces, whether Invisalign , Six Month Braces, or the Inman Aligner, might be a better choice. In simplest terms, I suggest thinking of it like this:

But First – Veneers/Crowns or Braces?

How Many Porcelain Veneers Do You Need?

Take another look at Matthew Perry’s smile above and you’ll understand why these 3 questions are critical:

How big is your smile? IOW when you look in the mirror and smile big, how many teeth do you see?
How much whiter do you want your teeth? A little bit or a lot? Or are you happy with the whiteness?
Besides the color, do you want to change anything about the length, width, shape, or overall size?

In simple terms, the bigger your smile and the bigger the changes that you want, the more teeth have to be treated.

How Many Teeth Show In Your Smile?

No – not just looking straight on – look from the sides, too. Too many people think they only show 6 teeth, because that’s what they see in the mirror. But if you turn your head just a little to one side, you’ll realize that you probably show at least 8 or 10 teeth total in a big grin. You see what a big mistake that was for Matthew Perry, right? He clearly needed 10-12 veneers, his grin is so wide. That’s why I also had to do 10 veneers for my wife, Fara, as seen below.

Of course, if you are only unhappy with 1-2 teeth in that whole smile, you always have the option of only treating those specific teeth. Just because you show a lot of teeth in your smile doesn’t mean all of them need porcelain veneers or crowns, but if you want to change your whole smile, you do need to plan on treating ALL the teeth that show in your smile.

How Much Whiter Do You Want Your Smile?

Again, Matthew is a great example. He obviously wanted his smile to be a lot brighter; he’s in Hollywood after all. It’s easy to see that, if you make a dramatic change in the color of your smile, you can’t stop half way. This is one of the most important reasons to look at how many teeth you show when smiling. If you want much brighter teeth, you have to do all the teeth that show. Sometimes that means doing the bottom teeth, too.

If you’re keeping the color teeth you already have, you have more freedom to choose how many teeth get veneers. In fact, if you like the natural color of your teeth, you might only need to do whichever teeth you don’t like. However, if you only get 1-2 teeth done but want your whole smile brighter, do Teeth Whitening FIRST. Porcelain can’t be whitened!

Are You Changing the Size, Shape, Length, or Width?

Let’s take my own teeth (Dr. Payet’s) as an example. These veneers were done in March 2006, just before Fara and I got married, by Dr. Steven Ghim . As you can see, my teeth before veneers were pretty ugly. I had braces for 3 years in high school and 2 years at age 28. After years of coffee and Coca-Cola, they were badly stained and demineralized. I also disliked the shape of my teeth and wanted a more masculine, squared-off, straight look. To get a bright white smile all the way across, meant I needed 8 veneers. See how my smile would have looked as bad as Matthew Perry’s if I hadn’t done all the teeth that show? Realistically, I also needed some minor gum surgery, aka a Gum Lift, to get all the gum levels and tooth lengths even. I just didn’t have time. When these veneers need replacing,I’ll get that done. I’ll also get porcelain onlays on my molars so they match, too.

Do You Need Your Wisdom Teeth Removed?

Given all the other nonsense floating around social media, I’m not surprised to see this item circulating how dentists allegedly rip off the public: wisdom teeth extractions . So let’s talk about the following questions, then cover the answers. Unlike most of my articles, this is going to be a lot of LISTS! I’ve also included a sampling of x-rays taken in my office within just the last few weeks as examples of many of these situations. At the end of the article, I’ll also provide a short debunking of the nonsense in this article that occasionally floats around FB and Twitter: Wisdom Teeth Dental Scam & Why You Need Your Wisdom Teeth (hint: you really don’t).

Even before getting to the reasons why wisdom teeth need to be removed sometimes, let’s talk about how dentists and oral surgeons generally diagnose and describe wisdom teeth, aka 3rd molars. The most common x-ray used to see wisdom teeth and figure out if they need to be extracted or not is called a panoramic x-ray . It looks like this:

Why Should Wisdom Teeth be Removed?

When Is it Better to Leave Wisdom Teeth in Place?

There are also scenarios in which you really don’t need them removed:

If they are fully erupted and you’re able to clean them normally to avoid cavities or gum disease.
If they are completely impacted and therefore can’t get get cavities or gum disease
If the tooth is so close to the big nerve in your lower jaw, called the Inferior Alveolar Nerve, or IAN, that there’s a big risk fo damaging that important nerve.
If the mandible (lower jaw bone) is at risk of breaking

Should You Get All of Your Wisdom Teeth Removed at One Time?

This is THE most common question that we hear, and it’s a very understandable one because of the nervousness that usually goes along with getting your 3rd molars removed. So here are my thoughts on the matter, and bear in mind – these are my opinions only:

If all of them are causing you pain, then it seems clear that getting them all done at once is going to give you the greatest relief the fastest.
If less than all of them bother you, what you do about the non-bothersome ones is really up to you.
If you’re already going in for 1 or 2, why not just get them all done? IOW, why wait until the others bother you later on and you have to go through it again?
If you’re choosing sedation, why not get them all done? You’re going to be knocked out once, why wait and go through it again?

As you can see, I take a fairly practical approach – if you’re already getting 1 or more done, and IF the others will have to be done at some point anyway, why put yourself through more than 1 appointment of numbing and discomfort? As the saying goes, “Just git ‘er dun.”

Obviously, if 1 or more of your wisdom teeth are impacted such that the risks are higher than the benefits, then only get the ones taken out that really need it.

Do You Need to be Sedated?

Many people do choose oral surgeons for removal of 3rd molars because they want to be “knocked out,” or sedated, and not many general dentists are licensed to do true sedation. The more difficult the extractions will be, the more likely you’ll want to be sedated. When teeth are impacted, that usually means they’ll be more difficult. There are 4 types of impacted teeth, and then these can be combined:

Soft-tissue: the crown, or top, of the tooth, has risen out of the bone but is still covered by gum tissue
Hard-tissue: the entire tooth is still covered by bone
Horizontal: the tooth is lying on its side
Vertical: the tooth is upright

In general terms, soft-tissue and vertical impactions are easier to remove while hard-tissue and horizontal impactions are more difficult , so those are the ones you’ll more likely want to be sedated. However, you should talk with your dentist or surgeon specifically to decide if you really “need” to be sedated, because many wisdom teeth can be removed so quickly and easily by a skilled surgeon that sedation would actually make your recovery take longer than necessary.

What are the Risks and How Common are They?

As with any surgery, there are always risks, and you should absolutely take the time to discuss the risks and benefits of removing your wisdom teeth. Sometimes the risks are too great, and ultimately you have to make the final decision to proceed. But the most frequent complications that occur after extraction of wisdom teeth are:

Dry socket
Excessive bleeding
Swelling of the face
Paresthesia or Dysethesia: Temporary or permanent change in nerve sensation to your jaw, lips, and tongue

Of all these complications, the 2 most common ones are Dry Socket and swelling of the face. The swelling can usually be taken care of with proper use of ice packs for the first 3 days, then with heat packs if needed after that.

And yes, as we often see in the news, it is possible to die during the surgery, such as in a recent tragic case in Minnesota that has been all over the news and social media. Fortunately, these are INCREDIBLY rare, and there is almost always an underlying medical condition. It is a point of fact that WAY more people die of dental infections every year than die from wisdom teeth surgery – not even close.

Do Dentists and Surgeons Recommend Extractions for Money?

Let’s get the obvious answer out of the way: we usually don’t do it for free, so that means we do make money when we take out your wisdom teeth. But do we recommend taking out wisdom teeth JUST to make money? In the HUGE majority of cases, the answer to that is a resounding NO. Of course, as with any profession that involves people, there are bad apples, and they give the rest of us a bad name. That sucks. But they’re few and far between.

To be completely honest, I HATE taking out wisdom teeth. Yes, I’ve removed about 3,000 teeth (not just wizzies) in my 17 year career to date (as of June 2015), and given the choice, I’d never take out another wisdom tooth again unless it’s super easy. Seriously. They’re way back in the mouth, often hard to reach, etc., and at this point, I almost automatically refer them out to my oral surgeon (see below for a list of the surgeons and periodontists with whom I have worked in Charlotte and can personally recommend. Don’t worry, though – there are a LOT of great surgeons in the Charlotte area, but it’s easiest to work with just a couple of them.) So in simplest terms, I make almost zero money from taking out wisdom teeth. Many general dentists feel the same way.

Do I get a “kickback” from the oral surgeons to whom I refer in the Charlotte area? Well, if you count the really big box of delicious cupcakes that they send us around Christmas or New Year’s as a “kickback,” I guess I do. Sometimes it’s cookies or cake, and almost always from a local Charlotte bakery, and our whole team really enjoys them. Heck, sometimes they’re all gone by the time I even know they arrived! But that’s it.

Can General Dentists Extract Wisdom Teeth as Well as an Oral Surgeon?

Simple answer: it depends. ��

I know some general dentists who are better than many oral surgeons, but that’s pretty rare. Oral surgeons get a TON of additional training and education, and they are both prepared and equipped to deal with all possible complications. That’s why most general dentists prefer to let the surgeons do it. I can extract easy ones it as well as the surgeons whom I recommend, but beyond that, it’s a different story.

Do You Need Your Wisdom Teeth?

Remember the article I mentioned at the beginning about a “wisdom tooth scam?” C’mon…that article, while based on real facts, makes some ridiculous errors of logic:

Just because you don’t “need” your wisdom teeth taken out, doesn’t mean that you “need” to keep them, either.
Just because doctors make money when they perform the procedure, does not mean that’s the only reason they doing it.
Just because you don’t like the reasons that the American Association of Oral & Maxillofacial Surgeons provides for taking out wisdom, doesn’t mean they’re not valid.

Here’s a quote that really bugs the heck out of me:

Wisdom teeth have their function in the body, or they wouldn’t be there. They are important for the chewing process and add to the biting surfaces. Dr. Jay Friedman

NO. Just NO. Wisdom teeth do NOT have some inherently important function in the body that means they need to stay there. That is just plain stupid. They’re just teeth and are NOT that important for the chewing process, because they are so far back in the mouth. If you try to chew with them, you’ll gag yourself at the same time. Very few people these days actually have enough room in their jaws for all of the wisdom teeth to fit, much less be used. This is what they usually look like. Do you seriously think that tooth is serving “some important function in the body?” DUH – NO!

Oh yeah, and about that “they wouldn’t be there if they didn’t have their function,” has this guy forgotten about the APPENDIX?

Simple answer? No, you do not “need” your wisdom teeth.

On the flip side, as long as they’re not causing problems for you, you don’t “need” to have them taken out, either. Many people live with them their whole lives just fine. The vast majority who get them removed have no significant complications and rarely miss more than a day of work, school, etc. At the same time, that doesn’t mean there are no serious complications, and for the people who get those, the recovery can be difficult.

So What Should You Do?

Think you need your wisdom teeth removed? Here’s what you need to know:

Make sure your dentist or oral surgeon has a high-quality x-ray that clearly shows where your wisdom teeth and other important anatomical structures are.
Ask the dentist or surgeon to clearly explain the risks and benefits of having YOUR wisdom teeth out; not some “general” wizzies.
Make sure to let the dentist or surgeon know ALL of the medications, herbs, and supplements that you are taking and that you completely inform them of any and all health conditions that you have. Do not ever lie on a medical form, and don’t assume that the doctor “doesn’t need to know about that.”
If you choose to proceed, follow ALL of the instructions given to you. If you’re not sure, CALL THE EMERGENCY NUMBER for the doctor.

And lastly, don’t forget that a Toothache Can Kill You , so if you do have a serious infection associated with a wisdom tooth, DO NOT IGNORE IT!

As I mentioned, we almost never do wisdom teeth extractions in our office. Here is a short list of surgeons and periodontists in Charlotte whom I trust and respect:

The most obvious reason that wisdom teeth should be removed is if they have a big cavity and would be difficult to fix because of how far back in the mouth they are. If you can’t keep them clean, you’ll just get cavities again. Save yourself the hassle now and get rid of them.
If they can only erupt partially and are still partially covered by gum tissue, that’s a great place for bacteria to grow since it’s very difficult to clean them. If the bacteria grow out of control, the infection is called pericoronitis (infection of the gum around the crown of a tooth) . This infection can be extremely painful. It can be easily treated with antibiotics, but if the tooth stays there, it can certainly happen again. Do you really want to leave it there and take a chance on going through THAT fun experience again? Exactly – I didn’t think so.
If they are impacted and resting directly against the back of the molar just in front of them, such that neither the gum nor bone can attach in that area. Again, those areas are bacteria and food traps, and sometimes cavities will form way down on the roots of the 2nd molars, and they can be VERY difficult to fix like that.

Do You Need a Crown on a Cracked Tooth?

We recently had a new patient see us with no tooth complaints, she just needed a check up and wanted to discuss her headaches to see if we could help. During her exam, I noticed that several of her molars were severely worn down (which she already knew about) and had cracks visible. We didn’t take photos at her appointment (should have!) to show her, but she seemed comfortable with my recommendation of doing crowns on those 4 teeth to catch and contain the cracks before the teeth break. Since she decided on a second opinion for those teeth, which I fully respect, I decided this is a good opportunity to review cracks in teeth, and what (if anything) to do about them.

One difficulty with diagnosing cracks in teeth, is that they almost never show up on x-rays. Why, you ask? Good question! It’s because x-rays are traveling perpendicular to the cracks, and the cracks are so narrow, there’s not enough change in the tooth density for the x-rays to show it. To show up on an x-ray, a crack in a tooth would have to be so wide, you could stick a fork in it, and then you’d already know there was a problem. The x-ray shown here is from another patient we recently treated, and while you can’t see anything on the x-ray, want until you see how it looked inside the tooth (on below).

This means we have to identify cracks with our eyes, and hopefully with magnification, because teeth are pretty small to start, and cracks are even smaller. That’s one reason we use dental microscopes in our office. But how do we know which cracks are worrisome, and which aren’t? The biggest problem is that – even if we can see them with our eyes, there’s no predictable way to see how deep they go, especially if there is already a filling in the tooth, as you’ll see in the following examples. amic x-ray . Tooth Cracks Don’t Show Up on X-Rays

Teeth Worn Down by Grinding Can Have Deep Cracks

People who clench or grind their teeth are at a high risk for teeth cracking. Enamel is a crystalline matrix, which means it acts a lot like porcelain (actually, that’s one reason we use porcelain for dental crowns). The longer your grind or clench your teeth, the more you stress that matrix, and eventually a crack starts forming. You’ve probably seen a plate or glass with a crack in it that seems to stay that way for a while, right? But then one day, it suddenly breaks, sometimes for no apparent reason. Well, that’s a pretty good analogy – cracked teeth can stay that way for a long time, but when they go, they go big-time, and sometimes the tooth is a goner, like this one from 10 years ago.

Three Cracked Teeth: 1 Crown and 2 Dental Fillings

So here’s our patient from yesterday, who’s teeth are a perfect example of what I see all the time (I have more than 3,000 photos of teeth with cracks, taken over the last 12+ years). As you look at the first case, which is getting a crown, you can see how worn down the tooth and old filling are. I’ve highlighted the deep crack with arrows so you can see how far it goes into the tooth – very close to the nerve!

The next 2 teeth both got fillings, but the first one could easily turn into a crown, too, because that crack went way up the side of the tooth. If she has any pain on biting, she knows she’ll need a crown. I felt there was enough strong, healthy tooth structure that a bonded tooth-colored filling to try that first. BTW – see all that dark “halo” around the old filling? That was a lot of decay from bacteria that worked their way around the filling. That’s part of the problem, too: while cracks look small to us, to bacteria they’re like a 4-lane highway. When I see a “double-crack” on the side of a tooth, I pretty much know there’s a cavity, too, just like here.

The last tooth (right next to this last one) had a much smaller crack that didn’t go nearly as deep into the tooth. This tooth will do just fine with a bonded filling.

And just to show that front teeth can crack, too, although this was on a different patient.

Wear a Nightguard to Prevent More Cracks

Certainly we always stress prevention over fixing things, so if you’ve been told you have cracks in your teeth, you should seriously consider a nightguard to take the stress off your teeth at night. I know that many people don’t believe they grind their teeth at night , but when you’re sound asleep, how would you know, right? Some people make noise, some don’t; some have sore jaws, others don’t – it varies a lot. But the simple act of wearing a nightguard will decrease the stress on your teeth and fillings, so they’ll all last longer with fewer problems. And look……..if your dentist tries to tell you how to save money, why wouldn’t you listen? LOL I promise, I make a lot LESS money on a nightguard than on fillings and crowns.

Do Teeth With Root Canals Need Crowns?

In “traditional” dentistry, the back teeth (premolars and molars) almost always need crowns to hold them together after a root canal , but for front teeth it all depends on how big the cavity was; since many front teeth still have a lot of healthy tooth left after the root canal, a crown is often not needed on front teeth. However, microscope-enhanced dentistry allows us to save more healthy tooth, so even some back teeth don’t need crowns after a root canal. However, you need to make sure and ask your dentist, because if you need a crown and don’t get it, the tooth will break and become non-fixable, which ends up being a waste of time and money.

A Dental Crown Holds the Tooth Together When a Lot is Missing

On the flip side, when the tooth has not been badly destroyed by decay, or not broken or cracked, a dentist with a microscope can do the root canal procedure with only a very small hole. With such a small hole, and most of the tooth preserved, we can avoid doing a crown and just do a simple bonded filling, and the tooth should still be fine for decades to come. Even if the patient needs more work down the road (the patient in the following photos is only 14yo), there will be a lot more healthy tooth to work with, than if I’d drilled it down for a crown.

Very often after a root canal, there’s simply not a lot of healthy tooth left over, and this is why many root-canal-treated teeth do need crowns. Here’s one example of a tooth we recently fixed with a root canal and a crown. The photo on the left is how the tooth looked after all the decay was cleaned out and smoothed (90% of this was done with our Lightwalker dental laser BTW), and the photo on the right is the “After” picture, once the root canal procedure was done, and after we’d bonded in the porcelain crown made the same day.

Can Cavities Be Cured?

I’ve seen a lot of posts and articles recently about whether or not dental cavities can be “cured,” with various holistic dentistry advocates pushing alleged cures such as colloidal silver, coconut oil, and some other stuff. We don’t see many people in our practice with these questions, but since it’s out there, I think it wise to address the issue head-on. Bear in mind – this article will pull no punches and might seem a bit offensive. My apologies in advance, but I am definitely getting on my soapbox here. ��

Update: I’ve written a new article that discusses the research that is asked about in the comments. There are ongoing studies into how to stimulate tooth growth with lasers, create gels of minerals and enzymes that could build new enamel, and more. As of 2016, however, any such research is at least a decade away from being available.

Holistic Dentistry Is a Scam and Hoax

Let’s look at tooth structure for the answer, because it’s quite simple: both dentin and enamel are “dead” tissues. So what does that mean, exactly?

Dentin (the layer under the enamel) is 70% minerals (hydroxyapatite), 20% organic materials (mostly collagen) and 10% water. You’ll notice that there are NO CELLS in dentin.
Enamel is 96% minerals (hydroxyapatite) and 4% water. Again – NO CELLS.
Basic human biology requires that there be cells that can reproduce, in order for any tissue to regrow or repair itself. No cells = no ability to regrow. Got it?

So by basic human biology, we know that teeth can not “regrow” in any way, because there are no “enamel cells” or “dentin cells” that can regenerate, which means they can’t heal themselves. So, we have that one out of the way, but now let’s deal with the other things these quacks claim can be done.

*** BTW – one really important point to consider when looking at most (not all, but most) of the “cures” promoted by holistics: for some strange reason, most of this “amazing research” that has been allegedly suppressed by mainstream science and dentistry comes from the late 1800’s and the early 1900’s, up until the 1950’s. Why is that, exactly? Personally, I have NO desire to have ANY of my doctors use medical research from 30-, 50-, 80-, or more than 100 years ago. I like my doctors using the most current and rigorous research, with the best equipment, scientific analysis, statistical analysis, etc. If any doctor told me that they were going to use a 100-year-old remedy on me for much of anything, I’d run screaming in fear. No thanks! ***

No sense in beating around the bush, eh? I’ll just get straight to the point and give you what I honestly believe, and tell you that there is virtually nothing espoused by “holistic” dentists that has ever been proven to be worth more than the paper it was printed on or the Internet space it took up. This holds true for the idea of “curing tooth decay,” too, as espoused on such junk “science” sites as NaturalNews.com or WellnessMama.com. Plain and simple, it’s true that Holistic, Alternative Medicine is Usually Wrong.

But getting specifically to the question about “curing” dental cavities, this post was largely inspired by this wonderfully thorough article that debunks the idea of healing cavities (particularly since the writer is not in the dental field, but did all the research on her own) on the FB Page I F********g Hate PseudoScience , which used to be one of my favorites – unfortunately, now it’s mostly clickbait, so this is one of the few good resources on it.

Home Remedies/Quack Dental Cures

So what are the other alleged methods for “curing” tooth decay? Here we go:

Oil-pulling : this is allegedly an ancient Indian remedy for reducing the number of bad bacteria in your mouth. Supposedly, rinsing with oils gets bacteria “stuck” in the oil and helps remove it. Quite frankly, there is no good science to support this. There have been some studies that showed a reduction in bacteria in people who rinsed with coconut or sesame oil; however, the same reduction was achieved faster with a simple chlorhexidine rinse. Heck – you can get that same reduction by swishing with Listerine, Crest, Colgate, or other mouthrinses. But is oil-pulling some magic remedy that will cure your cavities? Ummmmmmm………NO.

Diet: Some people have claimed that a diet high in Vitamin D can “cure” tooth decay. Hmmmmm, again, let’s look at biology.Allegedly, eating a diet high in Vitamin D gives your teeth the ability to create new dentin inside the pulp, which is somehow the tooth “healing” itself. Of course, if the cavity is coming from the outside, i.e. the enamel, I’m not quite sure how extra dentin on the inside qualifies as “healing,” but that leap of logic doesn’t seem to faze the believers. If the cavity is coming from the enamel, how is more dentin on the side “healing?” OK, it does provide some extra barrier, but there’s another problem with the idea….. what happens when the pulp runs out of space to put down new dentin? ’cause, you know, the pulp is pretty small to start, and if it’s getting smaller with more dentin, eventually it runs out of space, and once the pulp has squeezed itself out of existence, then what? Hmmmmmmm????? Dang, it’s stinks when logic interferes with quackery, doesn’t it?

Secondly, how exactly does Vitamin D. really get inside the tooth? Through the bloodstream, of course – it certainly can’t soak in from the outside, because it doesn’t stay there long enough. Ok, but how much Vitamin D. can you get into a tooth? Do you have any idea how small the opening at the tip of the root is? Trust me – it’s REALLY REALLY REALLY SMALL! Trying to get ANYTHING inside a tooth in a high enough concentration to do any good is almost impossible. And unless you can somehow force a VERY high concentration of Vitamin D. into the tooth, there’s no way it will do a dang bit of good. Again…….it sounds nice, but it simply doesn’t work. Somehow, though, the supporters of these methods conveniently ignore these basic facts.Oh, and one other “minor” detail: Vitamin D isn’t part of either enamel or dentin, so even if it gets into the tooth, it doesn’t do squat.

I often see statements that an “organic” diet will cure cavities. Ummmm, HOW? Organic simply describes a set of agricultural methods and rules, it doesn’t actually change the content of the food, and “organic” doesn’t magically make the food have anything in it that repairs cavities.

Same thing with “organic, free-roaming, grass-fed beef.” Again…..HOW? What ingredients are present in beef (of ANY kind) that magically repair a crystalline mineral matrix of calcium, phosphate, and fluoride? NONE. This is NONSENSE.

Don’t Want Cavities? PREVENTION

Interestingly, if you really get into reading some of the junk that’s out there, they also talk about using the same preventive techniques that dentists recommend, such as:

Avoiding foods high in sugars
Avoiding all highly-processed foods, especially carbohydrates
Brushing your teeth twice a day
Flossing regularly

You’d be hard-pressed to find a dentist who would disagree with any of these ideas, because they are all good ones. But none of the “studies” that are so often cited by the quacks seem to separate out any of them, so there’s no way to know if it was actually the brushing and flossing that was doing the trick, or the other stuff? Odd, isn’t it? Again, it’s that pesky logic that gets in the way, but logic seems to be in short supply among people who believe cavities can be “cured.”

Some Cavities Can Be Reversed

Look, there ARE some circumstances under which tooth decay can be reversed. When cavities are entirely within enamel, if a big enough change in the oral environment is made (eliminate excess sugars, acidic foods and drinks, etc) and a strong enough concentration of the right minerals (calcium, phosphates, and fluoride) can be applied and absorbed, those early-stage cavities can be remineralized, or re-hardened.

However, cavities found in the deep grooves of teeth are rarely reversible this way, because there is usually too much crud embedded in those grooves from your food, so it’s impossible to get the minerals in there. Cavities found between the teeth can be reversed, but it takes even more effort than small cavities found on the front or back of the teeth.

But once a cavity has penetrated through the enamel, it becomes impossible to reverse with current technology and techniques. It’s simply not possible to force a medication, or paste, or liquid of any kind that deep into the tooth to not only kill the bacteria, but to rebuild the mineral matrix.

There Are No Short Cuts

Look, ask any dentist, and s/he will tell you how wonderful it would be to find an easier way to fix cavities. With the methods we have to use today, patients really don’t like us, and do you really think we wouldn’t like some way to not have people tell us every day, “No offense doc, but I hate the dentist!” Sadly, there are no short-cuts, no easy fixes, no magical cures. We don’t like giving people shots, we don’t like the sound of the drill any more than you do, and we don’t like hearing people complain about us every day, when we’re just trying to help people get better.

But “holistic” dentistry? Just say no to junk science and quackery, PLEASE.

How Do I Know You Grind Your Teeth, Even if You Don’t Believe Me?

I don’t know why exactly, but patients at my Charlotte dentist office (and all around the country, according to my colleagues) seem to have a problem believing me when I say, “Wow – you grind your teeth really badly.” In fact, the most common response is, “No I don’t!” said with a passion that makes it seem I’m accusing them of killing baby animals. Seriously – and there are a few reasons why people think they’re right and I’m wrong……..even though I’m the doctor. Weird, huh?

OK………so what? News flash – your insurance won’t pay for a smile makeover, either! You tell me – do you want to pay me $5-800 for a nightguard, or do you want to pay me $40-50,000 for a smile makeover? Obviously, I make a lot more money with the smile makeover, but hey, there’s an easy way for you to NOT pay me for it – get the nightguard and wear it! It honestly and truly doesn’t make a difference to me which you choose, because it’s your choice and they’re your teeth. But if I’m telling you how to save tens of thousands of dollars down the road……isn’t it worth considering, even if the nightguard is out of your pocket?

Reasons People Don’t Think They Grind or Clench Their Teeth

Let’s get a little information out there so we have a basis for understanding what’s going on, shall we? This is a topic I really enjoy, so I try to stay reasonably up-to-date on current research and am friends with a fair number of internationally-recognized experts in the field. If you want to a more in-depth summary, this Wikipedia article on bruxism is mostly up-to-date.

Bruxism is classified as a SLEEP DISORDER. There is still a lot of debate about exact causes.
The “bite” has absolutely nothing to do with whether or not you grind your teeth. IOW, it doesn’t matter if your teeth fit together perfectly or all crazy.
If you do grind, it only happens in short bursts; it’s not all night. Each instance may only last for 30 seconds to a minute. You may well sleep with your mouth open 90% of the night (doubtful), but still grind the heck out of your teeth during the 10%.
You can clench up to 130% harder at night than during the day , because your protective reflexes are turned off.
Clenching and grinding aren’t really the same thing. Teeth Clenching : the teeth squeeze so hard that they can’t move. these patients typically do little damage to their teeth but have much worse pain.
Teeth Grinding : the teeth still squeeze hard, but not so hard that they can’t move around. These patients tend to do lots more damage, but actually have less pain.
Some people do a mix of both
Enamel is 15% harder than your bones; to wear your enamel flat in a lot of places, it takes a LOT of force, a LOT of times, over a LONG period of time. You can’t do it in a couple months.
An acidic diet (sodas, foods, etc) weakens your enamel and makes it easier to wear down during bruxism. It’s like a multiplier.
Not everybody grinds or clenches the same way, but everyone has a pattern that doesn’t seem to change. One of my colleagues and mentors tracked more than 400 patients over 10 years, and found that – no matter what treatment the patients had (braces, major reconstruction, combination), their bruxing pattern stayed amazingly consistent.

When patients tell me they KNOW they don’t grind their teeth when they sleep, I’m always a little amazed. I mean…. how the heck do you know what you do when you’re asleep? Here are some of the reasons people tell me how they know:

My husband/wife/significant other would hear me.
My jaws don’t hurt when I wake up.
My teeth don’t hurt.
I’m a light sleeper.
When I was younger, my dentist ground down my teeth to look like this.
I sleep with my mouth open.
I concentrate on sleeping with my mouth open, and that stops it. (Even better than just sleeping with your mouth open – you CONCENTRATE on it?)
I can’t grind my teeth all night, because I snore all the time.

There are probably a few others that I’ve heard, but these are the most common excuses. However, let me just remind you of one very important fact:

When you are asleep, you don’t have a clue what you’re doing or not, because you’re asleep!

So What Exactly is Bruxism, Anyway?

A Nightguard is Cheaper Than a Smile Makeover

Let’s look at some other examples of how teeth can look when damaged by bruxism (click on any image for a full-screen view). Now bear in mind…..NONE of these patients reported any jaw pain or headaches, and none of them believed that they did this by grinding their teeth, but I promise – there is no other way you can do this!

Now, looking at those photos, let’s talk about the cost of a nightguard (even a custom-made one by a dentist) vs. what it will cost to fix those teeth:

If you get a cheapo, “boil’n’bite” nightguard from the drugstore, it will probably only cost $25 or so. What’s the catch to the cheap option? I’ve never yet known anyone to actually wear one of those more than a week or 2. And if you don’t wear the nightguard, it doesn’t work, so what’s the point?
A custom-made nightguard at the dentist, like the NTI-tss Nightguard we recommend, can cost between $500-800; I have many patients wearing theirs now for more than 6 years with only minor repairs.
In comparison…… a smile makeover for any of the cases in that photo gallery will cost a minimum of $8,000, and potentially up to $40-50,000.

Why Dentists Tell You Different Things

If you’ve ever been to more than 1 dentist in a short period of time, for whatever reason, you’ve probably been a little surprised that one told you there were several problems, one didn’t say a thing, and maybe the third suggested something entirely different. This question has been discussed in several major news articles over the last couple decades, but usually with a major hidden bias or agenda that the public never gets to see, but which has a big influence on how the issue is presented. At our Charlotte NC dentist office, this is something we encounter on a regular basis, so why not address it head-on?

Dentists Are People, Too

In simple terms………dentists are like everyone else! And like people in every other field, that means there is a huge variety from which you may choose, such as:

Some are experienced but don’t stay up-to-date; others push the envelope of cutting-edge techniques
Some are young and driven to be the best; others don’t have a clue
Some love technology and buy every new gadget that comes out – others don’t know how to use email
Some are passionate about always learning more; others struggle to reach the minimum requirements to maintain their license.
Some understand research and base their recommendations on solid science, others are quacks and couldn’t read a research paper to save their life
Some dentists are more “aggressive” and some more “conservative”

You get the idea, right? This is no different than in any job – some people really want to be the best, others couldn’t care less, and then everyone in between Look at your co-workers, and you can probably figure out how to apply this list to them.

There Is Often More Than One “Right” Answer

So the hard part for you is to figure out what is the “right” recommendation, isn’t it? Well, let me throw another wrench into the gears – there might be more than one perfectly good recommendation ! This is usually because more than one treatment can resolve a problem; mostly the differences are in cost and time, but also sometimes they’re due to a dentist’s level of mastery or knowledge of a certain skill. Alternately, sometimes it depends on how well each dentist can see, as I discuss in this article about dentists and microscopes for awesome vision .

Some Answers are Definitely Less Good

On the flip side, just because there might be more than one option, that doesn’t mean that all of them are equally good, and some of them might be down right bad for you. By “bad,” I mean that they may not actually resolve your problem, or cost you significantly more than other options that would get a better or equally good result, etc, and some might truly make you worse off than you are now.

Different Philosophies of Care

Lastly, there is the fact that there are 2 basic “philosophies” of dental care , as I wrote about nearly 3 years ago, but let’s review. Bear in mind – these are generalizations, and most dentists fall somewhere in between these extremes.

If it ain’t broke or hurt, don’t fix it. This is a more old-school approach that basically doesn’t recommend any treatment until a tooth actually breaks or starts hurting. The benefit of this approach is you don’t pay for anything until it is obviously a problem. The big downside to this approach is that, once it’s broken or hurting, it’s usually more expensive and time-consuming to fix than if it had been caught early.
You’ve got a problem – let’s fix it now. This might be considered more “aggressive,” but usually isn’t. IOW, let’s catch and fix things when they’re small, because it’s less expensive to fix and you’ll spend less time in the dental chair. Think of it like this:

Small filling (small $) => medium filling (a little more $) => big filling (medium $) => crown (big $$) => root canal and crown (bigger $$$) => lose the tooth, get a bridge or implant (LOTS of $$$$) => etc etc etc……

Most Dentists are Good, Hard-Working People Who Care

So what’s the take-home message? Here you go:

Just because you get different opinions and fees from different dentists, doesn’t mean that one dentist is a crook and the other one honest. Just because they have different fees for the same thing, doesn’t mean the cheaper one is worse ; it doesn’t automatically mean the more expensive one is better, either.

Are there dishonest dentists? Of course! Just like in every profession, there are some bad apples, and sadly, they’re the ones you read about in the news. However, most dentists really care about their patients and want to do the right thing. But since dentists are people too, with different levels of training, experience, and dedication – it makes sense that you’ll get different opinions.

YOUR responsibility as a patient is to do your best to choose the right dentist for you. Ask questions, ask to see their work for other patients, ask them why they recommend something, and if it doesn’t make sense, ask if there are other options. If it still doesn’t make sense, get another opinion – or more than one. It’s ok – any good, honest dentist will have no problem with that and will actually welcome it.

Crowns Shouldn’t Make Gums Bleed

One thing that patients can’t stand, and understandably so, is getting a crown done, and after it’s put on, the gum around the crown turns red, sore, and it bleeds easily when brushing and flossing. After all, a crown should FIX a problem, not create one, right?

Why Do Some Crowns Make the Gums Red and Bleeding?

There are some important rules dentists must follow when doing crowns:

Make sure the lab (or CEREC Same Day Crown System, as in our case) has a crisp, clear edge so the fit of the crown is accurate;
Make sure the tooth is dry and not contaminated with blood, saliva, or water when the crown is cemented or bonded;
Make sure the final crown is the right size for the tooth; i.e. that it is not too bulky;
Make sure the edge of the crown is not too close to the bone, or it violates the “Biologic Width.”

Reason #4 is the most common reason for crowns to end up causing red, bleeding, and sore gums, but for patients, it is a harder to understand why, and why some minor gum surgery is really an easy and long-term fix to avoid an aggravating problem.

What is Biologic Width, and Should You Care?

Basically, you need a minimum distance between the edge of a crown and the bone. For most patients, for most teeth, it’s about 3mm. Sometimes a bit more, sometimes a bit less.

Now, if you put the edge of a crown closer than 3mm, your body doesn’t like it; it becomes a “foreign body,” and your immune system is triggered, thinking that it needs to get rid of that foreign body. Naturally, it can’t, but that doesn’t mean it stops trying, so you end up with chronic, sore, redness. And that is why, NO MATTER how much you brush and floss, the redness, bleeding, and soreness just will not go away.

So What? It’s Only Sore and Bleeding, Right?

Of course, even if that’s all it were, why go for years with an area that is always sore and bleeding? But no…..that crown can cause more problems, such as:

If you realize just how deep that edge is, you know it’s impossible to get your toothbrush and floss that deep to clean it;
With the edge that deep, it was probably hard for the dentist to keep the tooth dry and uncontaminated when putting the crown in, so the cement was probably contaminated on day 1 – that means it will fail sooner;
The immune system is going crazy to get rid of something it can’t, but the cells it’s using can actually destroy bone and gum attachment over time, so you lose healthy gum and bone;
An area that is always bleeding and irritated can get infected more easily and lead to a gum abscess or localized gum disease.

Why Would a Crown Be Too Close to Bone, Anyway?

There are several reasons this happen:

A big cavity that is very deep,
An old filling that then gets decay under it that is deep,
A tooth breaks below the gum line,
Some patients have too much bone too far on the enamel.

How Can It Be Avoided or Fixed?

If you have this problem already, you will need to be prepared to get the crown redone, even if it’s new. That’s the bad news. If your dentist anticipates it happening, though, this can be managed BEFORE the new crown is made, which is much easier.

The procedure is not really complicated, and it’s called Crown-Lengthening . It’s a minor surgical procedure that is usually done by a periodontist, or gum specialist, although some family dentists are trained in the procedure, too. In simple terms, it moves the edge of the bone away from where it was to create the space needed. A temporary crown is placed during healing (usually 3 months or so), and then a final crown is made that respects your gum and bone for total health and the longest-lasting result.

Teeth Whitening for Brown or White Spots

One of the most common frustrations experienced when people want to whiten their teeth, occurs when there are whitish and/or brown spots, because it can be very difficult to get the spots “evened out,” and with brown spots, it seems they just don’t whiten at all! Or do they…………? ��

What Causes Brown and White Spots on Teeth?

Besides the usual causes of tooth stains (coffee, dark sodas and juices, tea, etc), there are several causes of really hard-to-get-out tooth discoloration:

Excess fluoride intake as a child
Rheumatic fever as a child
Tetracycline antibiotics taken when young

Conventional whitening has difficulty with these stains, because they are deep inside the teeth. But can it be done, and how?

First of all, it can be done, but only by a dentist, because a strong acid must be used to open the enamel “pores,” so to speak, which will remove the dark mineral stains. The key, though, is that the right minerals must immediately be applied to replace what is taken out. We use a remineralizing paste called MI Paste Plus , which contains calcium, phosphates, and fluoride in just the right concentrations to remineralize teeth. We do this literally moments after the acid etch is washed off, so you don’t walk around with weakened teeth. Additional applications of the MI Paste are done by the patient every day, too, and the patient can even whiten their teeth, too.

Typically, this process has to be repeated several times, and the darker the teeth are to begin, the longer it will take and the more office visits are needed. But the great news is that it CAN be done!

Stacey was told she needed either bonding or porcelain veneers to get rid of these incredibly dark brown spots, but she didn’t want her teeth drilled, but regular whitening hadn’t worked at all. Waiving Dental Insurance Co-Pays is Fraud!

Fortunately, this is not a question I hear very often, but from colleagues around the country, I know that more and more patients are asking that their co-pays be waived, and more and more dentists are doing it. Heck, some dentists are even ADVERTISING it! But there’s something VERY important that every dentist and every patient should know:

Gold Fillings, Crowns, Onlays for Teeth – still the Gold Standard!

When asked what filling or crown material will last the longest, there is a very simple answer that has held true for over a century now, and it is still true today: Gold crowns, onlays, and dental bridges. Plain and simple, as much as I love doing cosmetic/esthetic dentistry with the ceramic materials available today, I still feel confident that NOTHING will last as long as a gold tooth restoration.

To tell the truth, I haven’t always felt that way, but I’ve come to that conclusion based on 11 years of seeing dental work that works over the long-term. For many years after graduating dental school, I never did gold restorations except in extreme situations where it was the only realistic material. But I have seen too many gold crowns, gold onlays, gold inlays, and even gold bridges that are 30-, 40-, or even 50+ years old! While very few dental materials have been in use longer than 30 years, of those, none have had the longevity of gold. So what are the advantages of gold?

It requires minimal drilling of a tooth to have sufficient thickness to be strong – only 0.5mm (trust me, that’s SMALL!)
It has the ability to deform slightly over time to match changes in adjacent teeth
It can be burnished, or “massaged” hard to the tooth edge to create an amazing seal against bacteria, sugar, acid, etc. Only in the last few years have porcelains reached equivalent seals, and even then the porcelains require absolutely exquisite bonding technique or they’re doomed to early failure.

So here is today’s example of a tooth with a failing silver/amalgam filling (decay around the edges and deep cracks that could have led to the tooth breaking at an inconvenient time) replaced with a gold onlay:

The gentleman who chose this is around 40 years old, and with regular dental care and the same excellent home care he has, this tooth and gold onlay will probably last until he is no longer in need of teeth any longer.

Why You Shouldn’t Ignore Cracks in Your Teeth

One of the most common reasons that patients come to us for dental emergencies is cracked and broken teeth. For some reason, this always seems to happen on weekends, when you’re out at dinner, or right after we’ve closed for a long weekend – go figure. This is avoidable, though, if you act preventively. Here’s the key, though, and this is one of the reasons we use digital photography so extensively to communicate with our patients — very often these teeth won’t hurt at all until they break. Don’t ask me why – no one really knows. But so often, what we’ll hear is, “Doctor Payet, it never bothered me at all, and then last night I was chewing on a banana and a big chunk of the tooth just broke off!”

Fix a Tooth Before or After It Breaks If It Doesn’t Hurt?

Depending on whom you see, different dentists will tell you different things . Many doctors still go by the philosophy of, “If it ain’t broke and it don’t hurt, leave it alone.” The problem with that philosophy (in my humble opinion) is that when it finally does break, it will take more time, cost more money, and until it’s fixed is often pretty painful than if it had been treated before it broke. If it’s a small crack, treat it with a nightguard and a filling. If it’s a big crack, treat it with a nightguard and a crown. But if it breaks, you might well need a nightguard, root canal, build-up filling, and a crown. And all because the small crack wasn’t treated with a filling.

Research tells us that teeth with tooth-colored (composite resin) fillings crack just as often as those with silver fillings. My experience over 15 years, however, tells me that teeth with LARGE silver fillings crack much more frequently than with composite fillings. But even the bonding process can’t stop a crack from forming if the stress isn’t relieved. I’d like to show you some examples of what we often see:

It does seem that these cracks form around old fillings most of the time……..but they can even happen in teeth that have never had any work done. All it takes is a lot of force (usually grinding and/or clenching your teeth) and enough time. Here’s an example from a young man in his late 30’s who clenches his teeth so hard that he split this back molar completely in 1/2. It had NEVER had any dental work, and in fact, it didn’t even have a cavity. But he split it so far that it had to be extracted. Obviously I recommended a nightguard to help him not do this to any other teeth!

So what’s my point? Simply this: if you have been told that your tooth is cracked, at the VERY least, please make sure that you have a nightguard, because clenching and grinding of your teeth is the most common cause. But also — get it treated BEFORE it breaks. It’s less expensive and a lot less uncomfortable that way. And you won’t have to worry about it breaking over that long weekend or holiday when we’re out of the office.
